ANNUAL REGISTRATION FEE: $25 per student / $50 per family

IMPORTANT: Registration requires a completed form and payment of the annual registration fee.
When you submit this form, its contents are emailed to the directors of Soaring Spirit Dance Studio.
Payment of the annual registration fee can be made online immediately after you submit this form.
